A. Business Documents
1. Businesses Articles of Incorporation

3. Businesses Corporate Certificate of Good Standing Must be dated within one year- Get from Secretary of State
B. Permits – please provide a copy of all the permits your project will need. If you have applied for permits please provide the status of each permit. Need permit information for
C. Location Map provide a location map GPS Coordinates which identifies the location of your business, must be detailed enough to identify the site on a flood plain map

D. Hazardous Wastes/Materials – describe any hazardous wastes or materials your business uses and a detailed description of how they are stored and disposed of and what if any local, state and federal regulations you must comply with.
E. Areal Photo - If the project involves agricultural land, please provide a photo from FSA.
F. Pictures - If the project involves structures over 50 years old please provide interior and exterior pictures from various angles.
G. IRS Documents – Please provide copies of IRS forms that apply to your business.

I. Matching Fund Documentation
Need a firm commitment letter from lender or funding source providing the matching funds. If available cash or savings is intended to be used as a match, an account statement showing sufficient available funds must be attached. Statement must be no older than 30 days of grant submittal date.
